Microphlebectomy, also called ambulatory microphlebectomy or phlebectomy, is a procedure that removes varicose veins through incisions in your skin. The procedure uses a phlebectomy hook, similar in shape to a crochet hook. This is the most important instrument for microphlebectomy.

How Does Microphlebectomy Work?

Microphlebectomy works by removing varicose veins using a phlebectomy hook. Very small incisions will be created on the skin to allow the hook to get to the targeted vein. After getting the vein, it will be pulled out through the incision to remove it permanently.

Is Microphlebectomy Effective and Reliable?

Microphlebectomy is highly effective for removing larger, bulging varicose veins that may not be suitable for less invasive treatments like sclerotherapy. This procedure allows for the direct removal of the veins, resulting in immediate relief from symptoms such as pain, swelling, and fatigue in the legs. It’s also a reliable procedure because of its widespread use and the predictable outcomes it provides. It is considered a safe procedure with a low risk of complications.

Am I a Candidate For Microphlebectomy?

You may be a good candidate for microphlebectomy if you have prominent, bulging varicose veins located close to the skin’s surface, particularly veins that are too large for sclerotherapy or unlikely to respond adequately to other treatments. Varicose veins should be removed to avoid developing blood clots. During your evaluation, Dr. Raffi Dishakjian will examine your veins and, when appropriate, use duplex ultrasound to determine their source, size, and location before recommending the treatment approach best suited to your condition.

What Issues Can be Treated with Microphlebectomy?

Microphlebectomy has been successfully used to treat varicose veins of all sizes at almost all parts of the body except saphenofemoral or saphenopopliteal junctions. Accordingly, microphlebectomy is usually performed to remove the very superficial veins that remain after treating saphenous vein reflux with surgery, or endovenous thermal ablation techniques, such as EVLT or Venefit. It is also the treatment of choice for the veins that appear on the dorsal aspect of the foot and around the ankles because venous surgery and Sclerotherapy are only suitable for special cases in that region. Even though microphlebectomy is a very time-consuming procedure, it is widely used in many countries because capital investment is the least with this technique. Results are also lasting and the cosmetic outcome is excellent, especially for the very superficial varicose veins. As with other vein treatment techniques, ambulatory microphlebectomy is most effective when the sources of reflux are identified and eliminated before removing any superficial vein.

The Microphlebectomy Procedure

Microphlebectomy is performed under local anesthesia in an outpatient office setting. Before starting the procedure, the patient is inspected in a standing position and the targeted varicose vein is traced on the skin with an indelible pen. After the patient lies down, local anesthesia is injected into the skin all along the varicose vein. The area is disinfected, and the hook is inserted into the very tiny puncture. This puncture is just above the vein with a 1 mm dermal punch. The needle size can also depend on the size of the vein to be removed. During vein extraction, the varicose veins are hooked and gently pulled out with the aid of a clamp until the vein breaks. The incision is then covered with a mildly absorbing dressing and a new incision is made 2 to 4 inches away from the previous one. Stitches are not needed. Finally, the treated leg is bandaged and a compression stocking is placed over the bandage.

What to Expect During Your Microphlebectomy Recovery

After microphlebectomy, patients are asked to walk around the office before leaving and can typically resume most daily activities right away. Compression stockings or bandages should be worn as directed for at least five days to support healing, while strenuous exercise such as running should generally be avoided for about two weeks. You’ll also need to protect the small incision sites from direct sun exposure for the first couple of weeks to help minimize visible scarring.

Potential Complications of Microphlebectomy

The incidence of complications following microphlebectomy is low. Scars at the incision sites would hardly be noticeable after a few months. However, scars may be visible when the vein removed has required a relatively large incision and the patient has exposed the area to the sun in the first couple of weeks following the procedure. Wound necrosis, unsightly scars, and infection do not occur, but the occurrence of hematomas is common. Rare complications of microphlebectomy include the appearance of spider veins near the sites of incisions, pigmentation, blister formation, and temporary or permanent nerve damage.
